Background: PACs are common after heart surgery. They often do not require intervention unless they result in unfavorable hemodynamics or cause other arrhythmia.
AEG: The AEG shows an early atrial signal. If a PAC occurs before repolarization of the AV node it will not conduct to the ventricle resulting in an early atrial complex without a following ventricular signal. This “blocked” PAC is demonstrated in the idealized figure above. If the PACs occur after AV nodal repolarization it will result in an early atrial and ventricular complex as seen in the above patient example.
Adenosine: PACs are not typically responsive to adenosine.
Atrial Overdrive Pacing: Overdrive pacing may suppress PACs, which typically recur after pacing has stopped.
